AllCare Plus Pharmacy Data Breach Settlement

Individuals whose personal information was compromised in the June 2022 AllCare Plus Pharmacy data breach.

AllCare Plus Pharmacy has agreed to resolve claims stemming from a June 2022 data breach that reportedly exposed a wide range of consumer information, including names, dates of birth, addresses, Social Security numbers, driver’s license numbers, health data, financial account details, and prescription histories.

Plaintiffs in the class action lawsuit alleged that the company failed to safeguard sensitive data and did not act quickly enough to prevent or respond to the breach. Though AllCare denies any liability, it will provide compensation and services under the terms of the settlement.

Under the Terms of the Settlement

Class members may choose from multiple forms of compensation:

• $50 alternative cash payment for those not claiming other benefits

• Reimbursement up to $750 for documented expenses such as identity theft losses, credit monitoring services, and professional fees

• Reimbursement up to $5,000 for extraordinary losses tied to identity theft or fraud

• Up to $100 for up to five hours of lost time related to addressing breach consequences, paid at $20/hour

• Two years of free credit monitoring and identity protection through IDX, which includes three-bureau monitoring, recovery services, and $1 million in identity theft insurance

To qualify, a valid claim form must be submitted by July 3, 2025. Class members may only choose one form of benefit—either reimbursement or the flat $50 payment.
